Ethanol Load (Auto-brewery)

Summary

This marker evaluates the capacity of your microbiome to produce ethanol (alcohol) endogenously from the carbohydrates you consume. A high capacity means your bacteria and yeasts are acting as an internal "micro-distillery," generating a constant trickle of alcohol that overloads your liver and can cause you symptoms of hangovers and chronic fatigue, even if you never consume alcoholic beverages.

Overview

The process of brewing beer or wine involves feeding sugars to yeast or bacteria so they ferment them and produce alcohol as a byproduct. Exactly the same biochemical process can occur inside your gut.

Certain yeasts (like Saccharomyces or Candida) and opportunistic bacteria (notably specific strains of Klebsiella pneumoniae) possess the perfect enzymatic machinery to perform this alcoholic fermentation. In a healthy ecosystem, ethanol production is minimal and insignificant; the liver filters and eliminates it effortlessly.

However, if you suffer from gut dysbiosis and your diet includes a high percentage of simple sugars or refined carbohydrates, these populations of fermenting microorganisms can grow beyond normal. Instead of fermenting fiber into beneficial fatty acids (like butyrate), they begin to produce toxic levels of alcohol 24 hours a day.

Health impact

The constant endogenous production of ethanol is a silent systemic aggression that primarily affects the liver and brain:

  • Non-Alcoholic Fatty Liver Disease (NAFLD): This is the most serious and recently discovered clinical impact. Your liver is forced to process this internal alcohol constantly. Science has shown that these bacteria act as a direct driver for the accumulation of liver fat and inflammation (steatohepatitis), mimicking the liver damage of chronic alcoholism in abstinent patients.
  • Brain Fog and Fatigue (Hangover Symptoms): Excess ethanol and its toxic byproducts (such as acetaldehyde) travel to the brain through the blood. This causes a constant feeling of "brain fog", difficulty concentrating, mild dizziness, lethargy, and waking up with a hangover feeling. In extreme and very rare cases, it can manifest as "Auto-Brewery Syndrome," where a person may test positive on a breathalyzer after eating pasta or bread.
  • Intestinal Permeability (Leaky Gut): Alcohol is a direct irritant to mucous membranes. Its uninterrupted production in the intestinal lumen "burns" and weakens the tight junctions of the intestinal wall, opening the door for toxins (like LPS) to pass into the bloodstream, fueling a cycle of chronic inflammation.

Technical report

The assessment of the ethanol load is performed precisely by Shotgun metagenomics, quantifying the global genetic potential of alcoholic fermentation pathways in the gut ecosystem.

Our bioinformatics algorithm tracks and quantifies the abundance of key gene families, paying special attention to bacterial and fungal alcohol dehydrogenase (ADH) enzymes involved in the conversion of pyruvate and acetaldehyde to ethanol. Taxonomically, we cross-reference this functional data with the abundance of saccharolytic yeasts and, very critically, with the level of the phylum Proteobacteria, specifically assessing the load of hyper-alcohol-producing (HiAP) strains of the species Klebsiella pneumoniae, as well as Escherichia strains.

Low/normal levels are associated with healthy fermentative metabolism focused on short-chain fatty acid production. Conversely, high levels suggest your ecosystem appears to be diverting carbohydrates towards the ethanol pathway. In this scenario, intervention is strict. It requires temporarily suspending the substrate (very low carbohydrate, simple sugar, and refined starch diet), assessing liver function markers (transaminases), and using natural antimicrobial therapies (such as oregano or berberine) or competitive exclusion probiotics to eradicate fermenting pathobionts, in addition to supplementing with liver protectants and glutathione donors (such as NAC).
